Licensed Practical Nurse (Primary Care)

  • Demonstrate the ability to vary the order and sequence of routines to accomplish care needs based on patients' condition, physician orders or established protocols/procedures.
  • Demonstrate the ability to observe, identify and respond to the patient's needs, objective/subjective symptoms reported or identified and is able to modify the plan of care or process to meet the emotional, physical and social factors exhibited by the patient.
  • Demonstrate the ability to implement and input data and orders into CPRS as required by facility protocols and policies.
  • Administer and document selected medication via parenteral, intradermal, inhaled, oral or topical routes per SLVHCS policy.
  • Reinforce educational needs of patients related to diagnostic tests, procedures, health education and personal health planning, disease prevention, health promotion and disease management.
  • Utilize motivational interviewing strategies when engaging in face to face or non face to face encounters.
  • Recognize and respond to emergency situations, anticipating emergent protocol and implementation of it and seek assistance of the RN and/or MD/NP/PA to initiate appropriate emergency intervention, as directed.
  • Actively seeks out educational opportunities to enhance nursing knowledge skills, sharing new knowledge gained with staff to improve and advance nursing practice.
  • Follow clinic and team routines related to the implementation of the Patient Aligned Care Team (PACT) model.
  • Provide patient care that is appropriate to the cognitive, physical, emotional, and chronological maturation needs of the patient group served.

TOUR OF DUTY:Monday-Friday, 7:30AMand 4:00PM including occasional Saturdays.

    • Basic Requirements:

      • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment; Individuals granted a license by a jurisdiction that does not require graduation from an approved school.
      • Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
      • Physical Standards: Must meet the physical standard in VA Directive and Handbook 5019.
      • English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

      Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

      Grade Determinations: A minimum of one year of qualifying experience as a GS-5 or fully meeting all performance requirements for the GS-5 LPN.
