The ideal candidate must be compassionate, collaborative, organized, have strong assessment & up-to-date clinical skills and thrive in a fast-paced environment caring for a variety of patients & diagnoses. In this position you will:
  • Drive throughout your work day to see patients in private residences, assisted living, and skilled nursing facilities within a designated region.
  • Assess, plan, implement and evaluate nursing care for the patient utilizing the interdisciplinary care team.
  • Demonstrate critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Utilize software programs for the maintenance of patient records and communication with team members

Position description

The LPN provides basic nursing care to assigned patients under the direct supervision of the registered nurse. The LPN collects data through observation and communicates information to assist the RN in patient assessment and care planning. The LPN is expected to deliver care in accordance with the patient care plan and standards of care. The LPN accepts delegation from the RN in meeting learning/information needs of the patient/family.


Currently licensed or eligible for licensure as a Licensed Practical Nurse in the state of Wisconsin, Minnesota or Iowa, depending on where they are employed. Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

We will only accept certificates of course completion from the following providers:

• American Heart Association (Title: BLS for Healthcare Providers)
• Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
• Military Training Network

Additional qualifications

One year LPN experience within the last five years preferred. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, nights, holidays, weekends, and on-call. Ability to adapt to unpredictable situations within the work setting. Graduate of an accredited school of practical nursing required.

License or certification

Current LPN license by applicable state requirements. Maintains current Basic Life Support (BLS) for Health Care Providers.

8:00 am - 4:30 pm varying days of the week, Monday - Friday.

Weekend schedule

No weekends. Weekday holiday rotation.
