LPN Intgrated Care Services (ICS)

The primary function of the Licensed Practical Nurse in Integrated Care Service is in support for the evaluation and coordination of Community Care referrals for Alaska Veterans throughout the state of Alaska in addition to lower 48 VA and Community facilities. The LPN will be an active participant on the integrative team to facilitate the continuum of care from referral to the Community and a warm hand-off to the Primary Care team. The LPN is knowledgeable regarding requirements of the Joint Commission in the outpatient clinic setting and VA Purchased Care.
The LPN in this position may also be involved in scheduling Community Care appointments in the community for patients who are having their care referred to the community sector to facilitate care being received within access standards.
The Licensed Practical Nurse (LPN) at the GS-6 level has the knowledge and the skills necessary for a focused effort and self-direction in utilizing out the most complex practical nursing skills providing age specific care for adult, older adult and geriatric patient groups possessing the ability and knowledge to promote independence, encourage self-care, involve family or designated care giver in care of patient taking into consideration age, ethnic origin, preexisting medical conditions and special needs of any given patient. This GS-6 position requires minimal supervision.
The LPN completes assigned duties and responsibilities which require experience in a broad clinical work experience with demonstrated skills which can result in the resolution of a wide range of nursing problems and a wide range of patient conditions. The LPN maintains patient confidentiality always; and guides others to protect patient rights and family rights.


Work Schedule: Monday - Friday 7:30 am - 4:00 pm (subject to change at the discretion of the department)

    • Basic Requirements:
      U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

      Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment; Individuals granted a license by a jurisdiction that does not require graduation from an approved school.

      Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.Physical Standards: Must meet the physical standard in VA Directive and Handbook 5019.

      English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

      Experience: Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
      Grade Determinations: A minimum of one year of qualifying experience as a GS-5 or fully meeting all performance requirements for the GS-5 LPN.
