Position description

The Medication History Specialist is to obtain and document in EMR, in a timely manner, the medication and allergy histories for patients admitted to inpatient status through the emergency department and direct admission. This information will be used by physicians and other health care providers to determine treatment plans, and will provide the basis for medication orders during and following the hospital stay. The Medication History Specialist must communicate effectively and courteously with patients and health care providers, using interview skills, listening techniques, medication knowledge, and available resources to develop a complete and accurate electronic list of medications and allergies for each patient. Medication History Specialists will also be responsible for performing other specialty functions, including but not limited to workload evaluation, position justification, continuing quality improvement, or other assignments in support of hospital and pharmacy operations, as assigned.


Three years of RN experience in an applicable care setting is required.All associate degree or diploma RNs must provide documented evidence of program completion of their baccalaureate degree in nursing within 6 years from the last day of the month of their RN start date.Beginning on January 1, 2016 baccalaureate programs must be accredited. If graduation did not occur within the last two years, one year of RN experience in an applicable care setting is required or, effective October 1st, 2017 one year of current LPN experience at Mayo Clinic is required.

Additional qualifications

Strong interpersonal and communication skills. Flexibility in duties and scheduling. Ability to perform diversified duties with time limitations with high degree of accuracy. Demonstrate strong organization skills and the ability to prioritize daily duties and utilize time wisely using problem solving and critical thinking skills. Self-motivated.

License or certification

Current RN license by applicable state requirements. Maintains Basic Life Support (BLS) competency. Additional specialty certification/training as required by the work area.

Day/Evening shift rotation, 8 hour shifts during the week

Weekend schedule

Every third weekend, 12 hour shifts
