Mental Health-Primary Care Coordinated Care Manager (Registered Nurse)

The Registered Nurse Coordinated Care Manager (RN-CCM) provides direct care to the Mental Health Primary Care (MH-PC) veteran population seen at the West Texas VA Health Care System (WTVAHCS). The RN-CCM assists in the delivery of evidence based care for primary care patients identified with a variety of emotional and psychiatric problems including depression, anxiety disorders, alcohol misuse, etc. The incumbent will be responsible for monitoring patients, providing education and support, influencing adherence to guidelines by providing "on-time, on-target" information to primary care physicians, and collaboratively making appropriate care decisions. Although this position is located within the Primary Care teams, the RN-CCM is considered to be an active participating member of Mental Health serving to meet the best interests of Veterans' well-being. Collaboration of care is accomplished with other members of the MH-PC Treatment Team or other interdisciplinary treatment teams through involvement in Primary Care daily huddles, weekly or monthly staff meetings.

The RN-CCM demonstrates clinical knowledge in the profession, provides consultation and guidance to colleagues, role models effective clinical practice skills, teaches or provides orientation to less experienced staff, develops innovations in practice interventions, and may provide clinical supervision if the position requires. The RN-CCM aids in the clinical program development by providing input into the program effectiveness for modification of service patterns. He/She may be assigned in settings where they have no access to direct clinical supervision, such as community-based outpatient clinics (CBOCs) or satellite outpatient clinics. He/She may be assigned to work with special patient populations with highly complex health or mental health problems.

Work Schedule: Typical work schedule is Monday through Friday, 8:00am to 4:30pm. Work schedule may include hours outside of this time frame to allow for extended coverage.

    • BASIC REQUIREMENTS:

      -U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      -Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
      -Must be proficient in spoken and written English.
      -Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
      Grade Determinations: Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows:
      Practice: practice, ethics and resource utilization;
      Professional Development: performance, education/career development;
      Collaboration; collaboration, collegiality;
      Scientific Inquiry: quality of care, research.

      Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

      Nurse I, Level I - Associate Degree or Diploma in Nursing with no experience.
      Nurse I, Level II - ADN or Diploma in Nursing or ADN or Diploma in Nursing and Bachelors degree in a related field or Bachelor of Science in Nursing (BSN).
      Nurse I, Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related filed and approximately 2-3 years of experience; or a BSN with approximately 2-3 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
      Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 2-3 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
      Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

      Salary: Applicants education and experience will be reviewed by a Professional Standards Board in determining the grade and salary of the applicant selected.

      *Preferred Requirement: The RN-CCM must have an Associate Degree in Nursing with at least 2 years of experience in a clinical setting. Of those 2 years, at least1year ofexperience should be within mental health and/or drug/alcohol settings.

      Physical Requirements: Work is principally sedentary, however some slight physical effort may be necessary.

      Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

      Receiving Service Credit for Earning Annual (Vacation) Leave: VA may offer newly-appointed Federal employees credit for their job-related non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which they earn annual leave.

      Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
