The City of Oakland offers a Mediterranean Climate, thriving Restaurant scene, LIVE Entertainment at numerous historic venues, stunning neighborhoods with views of the bay and large Redwood Parks and trails for bike/hike lovers.
FLOAT NP PACT/UC/WH will be delineated specific to the site and clinical area of practice, utilizing an approved "Nurse Practitioner Scope of Practice". Other specialized practices and procedures may be included when the NP, NM and collaborating supervisory physician deem appropriate by completing the applicable documentation for approval. An Interim Expanded Role Verification of Competency will be used to request new functions and procedures between biennial credentialing dates. Clinical Practice areas may establish "Group Expanded Role Scopes." Competency must be established for all functions and procedures. The NP may be assigned as to a Patient Aligned Care Team (PACT) in the Martinez Outpatient Clinic, Oakland Outpatient Clinics, or Fairfield/Mare Island Outpatient Clinics.The NP may be assigned clinical time in UC, Women's Health &/or Telehealth for periods of coverage, providing hands-on patient care and facilitating collaboration with the staff of all care teams. The NP will communicate with end users at other NCHCS sites to improve clinical advice and integration into the Patient Aligned Care Team processes.The PACT NP must meet all mandatory education requirements. The nursing process model and established approved telephone triage protocols will be used when providing nursing telephone advice. The PACT NP will have demonstrated the ability to work collaboratively and effectively with individuals. The incumbent must have the ability to navigate between various software and mainframe systems on a personal computer with an established operating system. The incumbent must have the ability to communicate effectively both orally and in writing, and possess effective interpersonal communication skills and telephone skills. The incumbent must have the ability to perform multiple tasks and work in a fast-paced environment. This position is composed of an education component, unit specific competencies, and a performance component.


Duties

Maintain an integrated delivery system through expansion of clinical practices.
Maintain operational efficiencies through emphasis on safe practices and national safety.
Actively implements Lean principles, supports quality improvement projects and promotes a culture of continuous quality improvement. Identifies symptoms and classifies them by acuity based upon collected data.
Demonstrates basic triage knowledge in making appropriate clinical assessments.
Relies on intuitive and systematic approaches (protocols) to make health assessments. Sorts, organizes group categories, compares, analyzes and synthesizes data about the client.
Demonstrates knowledge regarding VA Administrative regulations such as entitlements, health care access points and local policies and procedures related to these elements.
Educates and instructs clients on how symptoms can be treated.
Provides health teaching to clients as appropriate.
Contracts verbally with the client to carry out the treatment plan recommended and or advised within set limits.
Provide information about desired outcomes.
Performs documentation on all visits in accordance with VA policies and procedures.
Demonstrates flexibility with job assignments in order to meet patient care needs within the settings of Primary Care.
Identifies patients with 2 forms of identification.
Evaluates Veterans physical and mental health, family and social history to identify health care needs.
Orders and interprets diagnostic tests.
Prescribes medications in accordance with scope of practice.
Maintains expected work load panel of patients, assuring their continuity of care, health maintenance, and meeting performance measures area priority
Completes all relevant clinical reminders and participates in performance and quality improvement activities
Uses the Telephone Record Manager Plus (TRM+) and the electronic health record "CPRS" to document all patient triage, visits, evaluations, assessments, care plans; and orders, drugs and devices.
Collaborates with team members to provide continuity of clinical care and implements a treatment plan
Participates In medical and/or nursing research and quality improvement
Performs Telephone Triage as needed per staffing requirements.
Fosters the practice of nursing triage by using sound clinical judgment, patient teaching activities, addressing ethical issues that impact clients and staff and will demonstrate leadership and mentoring of staff, which may include ethics consultations.
Coordinate and implement patient care specific to the age of the patient population served (adult and geriatric).
Ensure that quality care is provided in an efficient and safe manner, consistent with the unit's standards of care.
Evaluates and uses analysis findings of group process to identify, analyze and resolve care problems.
Practice according to established policies, standards of care, and standards of practice, and is responsible to ensure that patient care and operations are compliant with all applicable local, VISN, or National Standards, policies, statutes and regulations.
Play a key role in the ongoing systematic evaluation and improvement of the quality and effectiveness of nursing care and services as they affect patient outcomes.
Initiate interdisciplinary projects to improve organizational performance.
Clinical expert and consultant: reviews quality of care for documentation, phone activity, case reviews, chart reviews, etc.
Identifies educational needs of staff and ensures that these needs are addressed. Encourages staff in team building.

Makes sustained contributions to health care by sharing expertise within and/or outside the medical facility.
Engages in implementing an educational plan to meet changing program or service needs for self and others.
Participates in nursing, medical center or Health Care System committees promoting task accomplishment through interaction and problem solving with group members and other stakeholders.
Provide leadership in identifying and addressing ethical issues that impact veterans and staff including initiating and participating in ethics consultations.
Use Inductive Reasoning - the ability to combine pieces of information to form general rules or conclusions. Practice Deductive Reasoning - the ability to apply general rules to specific problems to produce answers that make sense.
Collaborate in the establishment of and supports a system for recognition of accomplishments and achievements by employees.
Participate in the development of education programs to increase productivity and performance. Forecast new knowledge needs for changing practice environment and population groups, planning, implementing and evaluating strategies to meet those needs.
Assess and provide for own educational needs to ensure competence in current and future functional role responsibility in a dynamic health care environment.

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
      • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
      • Current DEA Certificate for controlled substances schedules II-V
      • Meet the requirements for VA Prescriptive Authority.
      • A current National certification in any of the following: Adult Nurse Practitioner or Family Nurse Practitioner.
      • A Master's degree or higher from an accredited school of nursing.

      Preferred Experience:
      • Two (2) years of increasingly complex clinical practice as a Nurse Practitioner; have evidence of leadership, motivation, and self-direction; have a comprehensive understanding of the assessment and evaluation requirements; be able to utilize databases to acquire clinical information, and work effectively with an interdisciplinary team.
      • Current proficiency in Basic Life Support BLS/ACLS through the American Heart Association (Required prior to assuming clinical duties and must remain current -VHA Directive 11-77)
      • Minimum of five years successful practice, Minimum three years acute / critical / MSU or extensive telephone advice work will be considered.
      • Lean Principles Training.
      • Women's Health experience



      Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
      • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
      • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
      • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
      • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
      • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
