Nurse Practitioner Lead Primary Care

The Nurse Practitioner for this position will be assigned to thePrimary CareHealth Care Group at the VA Long Beach Healthcare System in Long Beach, California.The Nurse Practitioner works as a member of a multi-disciplinary health care team, collaborating with other members of the team to provide medical care to veterans. The Nurse Practitioner is expected to demonstrate advanced competencies and skills in assessing the physical and psychosocial status of men and women patients. Provides comprehensive assessment, diagnostic work-up and clinic intervention, including health history, physical examination, and patient education to veterans; outpatients include both rescheduled and unscheduled veterans. Initiate appropriate referrals and consultation to other clinical services. Order and evaluate diagnostic tests as appropriate, recommend treatment and/or referral. Establish and coordinate a plan of care related to patients identified problems. Validate the effectiveness of previously implemented medical treatment. Initiate, evaluate, and change orders for medications. Include patient and family in planning care and establishing realistic measurable goals for health maintenance/improvement. Record/document health appraisal information, clinical assessments, treatment decisions, plan of care, ongoing management and therapeutic outcomes, order medications, diagnostic tests and referral consultations where needed for every patient encounter; recording will be accomplished using the electronic comprehensive patient record system (CPRS), and will be completed on the day of the encounter. Follow Performance Measure guidelines including completion of clinical reminders in CPRS; access and timeliness standards; prescription order entry, National VA Formula requirements and Customer Services standards. Work efficiently with patients, families, members of the multi-disciplinary team, and other VALBHS staff, demonstrates flexibility in responding to clinic demands, adhere to personal and professional obligations and responsible, practice and ethical standards, and sustain a work environment that is harmonious and customer service oriented. The Nurse Practitioner shall participate in the continuous quality improvement program. The incumbent is expected to be familiar with and comply with all national standards utilized at this medical treatment facility, including VA Performance Measures and Monitors, Joint Commission, and NCQA standards.

We are a 24/7 medical center, incumbents may be expected towork nights, weekends, holidays, and off-tour to meet our Veteran's needs.

      • Physical standards - All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.
      • Registration Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full active and unrestricted registration to continue employment with the VA
      • Educational requirements can be met as follows:

      (1) Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed: a) The National League for Nursing Accrediting Commission (NLNAC); or b) The Commission on Collegiate Nursing Education (CCNE). OR

      (2) The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. OR

      (3) In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration, will meet the requirement of graduation from an approved school of professional nursing.

      • Applicants must hold a Master's of Science in Nursing (MSN) degree, and be certified at the national level as a Family Nurse Practitioner, or Adult Nurse Practitioner.
      • Applicants musthave a current, valid DEA #

      The following requirements are preferred for this position:

      • At a minimum, applicants should possess experience as an RN that is determined to be sustained and consistently at or above an acceptable level of quality. This may include experience as a Graduate Nurse Technician (GNT) provided that candidate was utilized as a professional nurse and passed the State licensing (board) examination on the first attempt, and experience as Nurse Technician Pending Graduation provided the candidate possessed an active, current registration to practice nursing in a State and was utilized as a professional nurse. Professional nursing experience should be documented on the Application for Employment Nurse and Nurse Anesthetists (VA Form 10-2850a).
