Duties

This position will serve as an intermittent Nursing Assistant in Nursing service. The incumbents will work at the Veterans Affairs Southern Nevada Medical Center, Las Vegas, Nevada. Incumbents can work a variety of shifts in a variety of departments including, but not limited to, 6th floor (GE Medical Unit or 6 W-Surgical Unit), ICU/SOU, Emergency Department, Primary Care, Specialty Care, and 2E Behavioral Health Unit. These positions report to the Office of Nursing Service through the Associate Nurse Executive/Community Care Services. The incumbent is expected to provide assistance to other clinical areas of the hospital due to volume of workload and or staffing issues.
Duties of the Nursing Assistant will include and are not limited to:

  • Identify and report data relative to more complex patient situations including subtle changes in patient behavior or condition or variance related to neurological signs, vital signs and other parameters.
  • Monitor intake and output, vital signs, percent ingestion of meals, and other activities of daily living
  • Document in computerized patient record system and or required paper forms and assures accuracy and completeness
  • Assist patient with transfers from bed to chair, bed to bedside commode using proper body mechanics to ensure safety of patient and self.
  • Provide 1:1 observation for homicidal or suicidal patients, patients who are a danger to self or others or due to a high fall risk as directed by the Nurse Manager, Charge Nurse or designee
  • Escort Veteran to Mental Health Court held at Rawson Neal Psychiatric Hospital
  • Provide and remove patient meal trays as appropriate, remaining cognizant of patients with special diets or who are ordered to not receive anything by mouth.
  • Assist patients with activities of daily living, promoting independence and autonomy, including but not limited to bathing, bowel and bladder care, eating, and positioning for comfort.
  • Work as a transporter, transporting patients from unit to testing and back to unit and or assisting patients from car to clinic, etc.
  • Assist and chaperone physician during female patient clinic exam.
  • Monitoring patient flow, identifies existing and potential problem areas of delays in processing a Veteran's medical care request.
  • Initiate action with the clinical staff to expedite the care and ensures the smooth flow of patient care from reception through disposition
  • Monitor environment of care during 15 minute rounds and initiate corrective action to prevent patient injury, reporting problems and resolution to Nurse Manager, Charge Nurse or Designee.
  • Report observations of the condition of acute and chronic phase patients such as excessive vomiting, bleeding, type and amount of drainage, skin integrity, patency of airway, etc.
  • Answer patient call lights, in person or electronically, ensuring patient is passed to appropriate nursing staff
  • Accurately communicate a variety of physicians orders involving patient activities and route information to appropriate departments.
  • Applies established guidelines concerning the release of information on patient condition
  • Demonstrate flexibility, integrity and accountability with a work ethic that emphasizes excellence and quality

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Education - One full year of education above high school with courses related to the Nursing Assistant Occupation. Completion of an intensive, specialized, occupation related course of study of less than one year as a Nursing Assistant may also be accepted.
      • Proficient in spoken and written English.
      • Experience - One full year of directly related work experience with progressively responsible assignments and experience equivalent to the GS-04 grade level which demonstrates knowledge, skills and abilities that are directly related the the specific assignment.
      Preferred Experience:
      • Certification as a Nursing Assistant by a State, Territory or Commonwealth of the United States of America

      Grade Determinations: The GS-05 level is the full performance level for Nursing Assistants. Nursing Assistants at this grade level function as a member of the nursing care team and assist licensed nursing staff in the care of patients/residents receiving outpatient care, home or community living care or other patients/residents who are in acute, sub acute or chronic states of illness. The distinguishing factor is that patient/resident assignments typically involve more complex nursing needs which can vary within a range of predictable to unpredictable requirements. Nursing Assistants at this level also assume more of a peer education/mentorship role. Nursing Assistants at the GS-05 level must be able to demonstrate professional knowledge, skills and abilities below:
      1. Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse.
      2. Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new nursing assistants by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting.
      3. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc.
