VA Northern Indiana Health Care Systems is larger than just a single facility. It is a fully-integrated medical care facility with a dual campus and Community Based Outpatient Clinics, (CBOCs), in Goshen, Mishawaka, Peru and Muncie, Indiana and combined VA mission. The combined mission of the VANINHCS means military retirees, and eligible veterans receive health care at these facilities.

This position is located within Social Work Service and can be filled at the Fort Wayne Campus or Marion Campus.

Duties

The Caregiver Support Coordination/Community Nursing Home Nurse has a split assignment that is roughly .75 Caregiver Support Coordination and .25 Community Nursing Home Program. In the Care Giver Support Coordination role, the nurse assists in the development of the caregiver support program at the facility level; clinical evidence-based services and interventions for the caregiver and Veteran; education and consultation for the caregiver, Veteran, and staff on caregiver issues; community outreach; resource development; continuous quality improvement activities; and the evaluation of caregiver issues. As the Community Nursing Home Nurse, the incumbent ensures that safe, high-quality care is provided to CNHC Veterans, provides nursing case management; in collaboration with the CNHC Social Worker, to address any non-routine medical, psychiatric, or prosthetic needs; helps facilitate outpatient VA evaluations, referrals, and/or admissions; and problem-solves any deficiencies or complications in patient care. In addition the incumbent must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations. The incumbent is versed in the clinical programming and daily operations of the program, as well as, for the management of data related to the program at the facility level.

The incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The incumbent is responsible for assisting in developing and establishing linkages and collaborate on: 1. VA caregiver support efforts with community healthcare and social service agencies . 2. VA contract community nursing home care efforts with community nursing homes and adult day health care agencies. The incumbent is also responsible and accountable assisting in the clinical and program development and effectiveness of both programs and therefore, the incumbent possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources as well as understanding the services and benefits available for contract community nursing home care.. The incumbent will conduct In- Home Evaluations required for the Comprehensive Assistance for Family Caregivers Program. The incumbent will perform bi-monthly and annual inspections of all contract community nursing homes.

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
      • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
      Preferred Experience:
      BSN
      2-3 years of clinical nursing experience


      Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
      • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
      • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
      • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
      • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
      • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
