Registered Nurse (Patient Flow Coordinator)

The Patient Flow Coordinator is responsible and accountable for the management of patient flow throughout the medical center to optimize patient care, patient safety and operational efficiency. The Patient Flow Coordinator is responsible for collaboration with administrative and clinical staff in actively managing and planning for actual and potential overflow or patient surge situations. The Patient Flow Coordinator is responsible for development of strategies to actively manage patient flow to protect patient safety during overflow situations. He/she will monitor bed capacity, staffed beds and the demand for beds daily keeping Service Chiefs apprised of any situation requiring implementation of Service action plans for discharges and elective admissions or the potential need for diversion. The Patient Flow Coordinator will collect and analyze data related to patient flow, including Length of Stay, Readmission Rates, transition times and report findings and make recommendations for improvement, as indicated. He/she will serve as Chair of the GCVHCS Flow Committee and ensure committee functioning to meet with VA National Directives. The Patient Flow coordinator will also serve in the role of Bed Management System (BMS) administrator.

Work Schedule: Day Shift with rotations as needed

    • BASIC REQUIREMENTS:
      • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
        • The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
        • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

      Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
      • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
      • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelors of Science in Nursing (BSN) with no additional nursing practice/experience.
      • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelors degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
      • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelors degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
      • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 years of nursing practice/experience.

      Preferred Characteristics/Experience: In addition to the basic qualification requirements, applicants must have demonstrated evidence of the following knowledge, skills, and abilities determined to be essential for satisfactory performance in the position.

      • Minimum of three (3) years of recent (within past 5 years) full time clinical experience as a RN in Inpatient Setting.
      • BSN Preferred
      • Proficient experience with Microsoft Word, Excel, and PowerPoint with focus on excel designing spreadsheets and data charts.
      • A high degree of personal and professional accountability, maturity, stability and the ability to function independently and work efficiently as a team with other personnel at all levels in the organization.
      • Ability to adapt to changing work assignments as needed to meet facility needs.
      • Ability to oversee, direct and manage staff and flow of patients in an inpatient setting.
      • Experience as a charge nurse or in a supervisory role.
      • Effective oral and written communication.
      • Exceptional internal and external customer service skills.
      • Experience with data review analysis and planning for Process Improvement related to data.
      • Bed Management System experience.
