Masters/Advanced Degree in Nursing Required
RN with Strong clinical experience/background in  Critical Care, and/or Emergency Department Care is a huge plus.
Ambulatory Care experience/familiarity is a plus.
RN with strong communication skills strongly preferred, as the RN will need to effectively communicate telephonically with physicians, patients, and other clinical staff.

Evaluates and coordinates the flow process involved in taking calls, facilitating acceptance and triage throughout the Physician Access Center. Acts as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into and throughout the Health System.  Provides clinical expertise, close medical communication and quality assessment for the successful referral, transfer, and overall care of patients.

1. Communicates with Medical Group, Voluntary and Community Physicians to help facilitate appropriate patient referrals in a timely manner.

2.  Provides telephone patient triage and physician outreach to coordinate proper patient healthcare.

3.   Triages clinical needs and coordinates clinical care for patients.

4. Communicates with referrers including hospital administrators, physicians, support staff, patients and families.

5. Processes, tracks documents, and directs patient care throughout the Health System.

6.  Maintains documentation for patient inquiries.

7.  Collects pertinent insurance information and matches for physician participation.

8. Promotes relationships between physicians to enhance physician involvement with Health System and for best practice patient care.

9.  May review patient’s prescriptions and medications, as needed.

Performs related duties, as required.


Qualifications

·        Master’s Degree in Nursing or related field, required.

·   Current license to practice as a Registered Professional Nurse in New York State, required.

·         Minimum of five (5) years clinical experience, required.

·         Demonstrated leadership, interpersonal and problem solving skills, required.

            Working knowledge of Microsoft applications, required.
