Duties

The RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Primary Care, Specialty healthcare teams and the larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN Care Manager is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN Care Manager demonstrates leadership by serving as an advocate for patients, team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Meets all mandatory requirements for assigned area and will perform activities that reflect the educational, experiential, and competency requirements outlined in the 4 Dimensions and the criteria identified under each Dimension.

The RN performs, reports, and records assigned nursing care in the Primary Care Clinics as described below and any other duties assigned by management.

  • Assesses patients for suitability of placement on Telehealth and appropriate levels of technology based on patient needs and fiscal considerations.
  • Actively recruits patients with high use, high cost consumption patterns and those at risk for long-term institutional care.
  • Enrolls patients utilizing approved disease management protocols for their clinical condition and reason for enrollment.
  • Uses appropriate telephone etiquette for both internal and external customers
  • Uses telephone assessment skills to identify actual and/or potential health, safety, and educational needs of the patient
  • Performs online monitoring of clinical parameters
  • Appropriately makes patient referrals for additional services based on caregiver and patient needs.
  • Appropriately prioritizes the urgency of patient needs utilizing critical thinking skills and decision support tools
  • Develops and documents a collaborative plan of care with patients, their support systems, and their provider upon enrollment and during periodic comprehensive reassessments.
  • Enters as ordered, locates as needed, and obtains results of x-rays, tests, procedures, and labs
  • Engages and documents patient education
  • Incorporates knowledge of ethical/legal issues and professional nursing standards necessary for providing patient care and information
  • Knowledge of and implementation of PACT principles
  • Ensures continuity of operations for assigned patients and ensures emergency planning is discussed and an appropriate category of care and acuity level is assigned and documented in CPRS.
  • Monitors patients for adherence to program requirements, and ensuring non-responders and partial responders are managed according to OTS guidance
  • Facilitates and acts as provider in RN clinic including providing knowledge/information to patient/family to promote self-care of acute/chronic disease management
  • Applies clinical knowledge of primary health care in prevention and wellness, health risk assessment, disease management, and self-care
  • Adheres to proper infection control procedures regarding equipment, cleaning, and storage
  • Understands responsibility for the safe processing of reusable medical equipment
  • Follows policies for insuring the correct patient is receiving the correct treatment by using two patient identifiers
  • Performs procedures as directed
  • Administers medications as ordered and documents appropriately
  • Follows policies/procedures developed by their service, facility, and VISN
  • Follows program specific Conditions of Participation (COP) for VHA Home Telehealth Programs.
Work Schedule: Monday-Friday, 7:30 am-4:00 pm
There are two (2) vacancies for a Registered Nurse in Telehealth. One position will cover patients in the Indianapolis area, and the second will cover patients in Wakeman, Bloomington and Martinsville areas.

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
      • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
      Preferred Experience:
      • BSN preferred
      • 5 years of nursing experience
      Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
      • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
      • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
      • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
      • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
