Responsibilities: The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions. Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected. (012560)

Qualifications: Graduate of a baccalaureate Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience.

Additional Qualifications: Looking for a self-directed nurse to assist in building the care team model. Demonstrated leadership, effective communicator, proficient computer skills, and excellent critical thinking skills. Bachelor of Science in Nursing required. Hematology/Oncology/BMT or Ambulatory experience preferred.

License or Certification: Must obtain prior to start date:

Current RN license by applicable state requirements.
Arizona and Florida: Maintains Basic Life Support (BLS) competency.

Midwest: Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

We will only accept certificates of course completion from the following providers:
  • American Heart Association (Title: BLS for Healthcare Providers)
  • Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
  • Military Training Network

Additional specialty certification/training as required by the work area.

Exemption Status: Non-exempt
Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $32.72 per hour.
Hours/Pay Period: 72 hours per 2 week pay period
Schedule Details: Monday - Friday; Days, 8-hour shifts - NOT NEGOTIABLE
Weekend Schedule: No weekends
