RN- Case Manager - Vascular Surgery

The Vascular Surgery Registered Nurse (RN) Case Manager provides care coordination and management in an inpatient and outpatient setting with acute and chronic vascular surgery patients at the VA Palo Alto Health Care System (VAPAHCS). Vascular Surgery performs procedures in both the main Operating Room (OR) an in radiology (Interventional Radiology/IR); each has its unique requirements for care coodination. The surgical procedures in the main OR typically have an open compenent while cases in IR are endovascular procedures. He/She works collaboratively with the attending physicians, surgical house staff including residents and fellows, the Vascular Surgery Nurse Practitioner, other nursing staff, interdisciplinary staff, patients, and their families to ensure coordinated care for Vascular Surgery patients.

The major duties and responsibilities include, but are not limited to:

  • Be the primary point of contact for Vascular Surgery patients once they are indicated for a Vascular Surgery procedure.
  • Schedule and coordinate pre-operative visits and studies (CT scans, ultrasounds, etc.) and any other unique needs required by the surgical patient.
  • Coordinate the weekly operating room schedule to include: contacting patients, arranging for pre-operative work-ups, surgery scheduling, arranging for appropriate peri-operative care and arranging for lodging reservations as needed.
  • Instructed in treatment of life threatening situations where a physician is not immediately available.

    • BASIC REQUIREMENTS:

      Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by the National League of Nursing Accreditation Commission (NLNAC) which changed to the Accreditation Commission for Education in Nursing (ACEN; http://www.acenursing.org ) or The Commission on Collegiate Nursing Education (CCNE; http://www.aacn.nche.edu/ ) at the time the program was completed; OR the completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

      Current, full, active, and unrestricted registration (i.e., licensure) as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or the District of Columbia. (Note: Provide your RN registered license number on your application.)

      In addition to the basic requirements, the following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

      Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

      Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

      Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

      Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

      Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

      Business Necessity:

      • One (1) year of full-time experience as a licensed professional nurse.

      Preferred Criteria:

      • Current BLS provider card issued by American Heart Association (AHA) or Military Training Network (MTN).
      • One (1) year ofCase Managementexperience.
      • RN experience in an inpatient and outpatient setting with acute and chronic vascular surgery patients.

      Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
