General Summary StatementProvides comprehensive, quality nursing care through application of the nursing process. Adheres to Nursing Policies and Procedures and Organizational Goals and Objectives. Essential Duties and ResponsibilitiesClinical Practice1.\tProvides age appropriate care for the population served. Gives consideration to growth & developmental phase of life, to age related factors such as thermal regulation, nutrition, medication administration, nutrition, safety and functional needs of the patient.2.\tUses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs.3.\tEnsures attention to patients physical, spiritual and emotional well being.4.\tDemonstrates competency in assessment skills (Includes admission and shift assessments).5.\tPerforms pain assessments and intervenes to optimally meet patients needs for comfort.6.\tPlans clinical care based on patients needs and uses available resources.7.\tPerforms clinical care in accordance with policy and procedure.8.\tPerforms peripheral venipuncture and peripheral and central vascular access care according to policy & procedure.9.\tEvaluates outcomes of nursing interventions and takes appropriate actions to move patient toward reaching goals of care.10.\tIdentifies situations that warrant patient rescue and takes appropriate action, e.g. Rapid Response or Code Blue/Pink.11.\tAdheres to the 5 rights of medication administration and uses the Medication Administration Check (MAK) system per policy and procedure.12.\tIntegrates principles of infection control & infection prevention into practice.13.\tAdheres to aseptic technique as appropriate.14.\tUses resources and references for reviewing policies and procedures, medications, and clinical information.15.\tPrioritizes and organizes work effectively within assigned shift.16.\tAdheres to CCH Joint Commission Disease specific guidelines and core measures for the care of the patients17.\tImplements nursing process for the patient who presents with complex medical needs.Communication1.\tUses effective communication during hand-offs. (e.g. Shift report, transfer report, etc).2.\tOrganizes information into a meaningful format, such as SBAR.3.\tDelegates appropriately to unlicensed personnel and team members.4.\tCommunicates patient care information to the interdisciplinary care team. 5.\tProvides patient and family education in a manner that is understood and gives the patient and family handouts, brochures and resources that can be taken home for future reference.6.\tUtilizes principles of Team Effectiveness and Commitment to my Co-WorkersDocumentation1.\tDocuments patient care in an accurate, professional and timely manner using Soarian, MAK, and paper documentation forms according to policy and procedure.2.\tDocuments patient care that is relevant to patients diagnosis, problems and reflects patients status.Professional Role Development1.\tDevelops own nursing knowledge via practice, observation, professional literature, networking and attending continuing education programs.2.\tPromotes best practice by demonstrating a commitment to quality patient care and the profession of nursing.3.\tPromotes innovation and creativity in the practice environment.4.\tServes as a preceptor and mentor for new employees.5.\tMentors student nurses.6.\tAccepts constructive feedback about performance or behaviors and shows evidence of change.7.\tDemonstrates verbal and writing skills that are appropriate for healthcare communication. 8.\tAdheres to the Department of Nursing Dress Code.Department of Nursing and Organization Support1.\tAttends assigned committee meetings and contributes to the committees. 2.\tCommunicates support of organizational values and strategies.3.\tSupports the following organizational programs: Magnet®, Hospital of Distinction, Joint Commission Disease Specific Certification, Nursing Councils, and integrates their standards into clinical practice.4.\tReviews nursing quality data, e.g. NDNQI and identifies how to apply best practice into own practice and mentor others towards meeting quality target goals.5.\tMaintains attendance/lateness standards in accordance with Hospital policy.Departmental Support1.\tMaintains equipment in working order. Uses equipment appropriately, follows Department policies for equipment and supply use and maintenance.2.\tPerforms assigned duties to maintain unit in a state of survey readiness and maintain quality initiatives. For example, quality control measures, emergency equipment checks, medication preparation area cleanliness, etc.3.\tAdheres to unit programs to support patient care such as maintaining a quiet environment, hourly patient rounds, customer service, etc.4.\tAttends staff meetings on a regular basis.5.\tKeeps patient care area and work environment neat and organized.

Education Training and Experience\nGraduate of an accredited Nursing Program. Bachelors of Science in Nursing (BSN) is required. \n\nExperience preferred, however training and specialty specific education programs are provided during orientation, and must be successfully completed as scheduled.\nBasic computer skills: keyboard, mouse use. Ability to complete data entry on clinical documentation screens. Ability to use email and internet. Ability to successfully complete on the job training for CCH computer programs. Ability to practice critical thinking and organizational skills. Ability to speak, read, write and understand English.\n\nLicensure/Certification \nCurrent Pennsylvania RN License. \nCPR certification required. Certification in clinical specialty is preferred.\n\nMust meet specialty specific competency requirements per the job description.\n\nAmbulatory Care/Pre-Admission: No Additional \n\nAntenatal: AWHONN fetal monitoring program completion required.\n\nEmergency: ACLS, PALS certification required, or must complete within 12 months of hire.\n\nEndoscopy: ACLS, PALS certification required, or must complete within 12 months of hire.\n\nICU, Critical Care Support, PINU: ACLS certification required, or must complete within 12 months of hire.\n\nL&D: NRP and ACLS certification required, or must complete within 12 months of hire. Completion of a WHONN fetal monitoring program within 12 months of hire.\n\nMed-Surg (including Med-Surg IHR): No Additional\n\nMed-Surg (WWG): ACLS certification required, or must complete within 12 months of hire.\n\nMother-Baby: NRP certification required, or must complete within 12 months of hire. STABLE program completion is preferred.\n\nNICU: NRP certification required, or must complete within 12 months of hire. STABLE: must be completed with 12 months of hire\n\nOperating Room: Two years’ experience in the operating room or perioperative credit course is required.\n\nPACU: ACLS and PALS certification required, or must complete within 12 months of hire.\n\nPediatrics: PALS certification required, or must complete within 12 months of hire.\n\nTelemetry: ACLS certification required, or must complete within 12 months of hire.\n\n\nWork Environment and Hazards\nDirect patient care areas on nursing unit. Risk of exposure to potentially hazardous materials and body fluids. May become involved in crisis situations.\n\nWork Contact Group\nThe Chester County Hospital personnel, physicians, volunteers, patients and their families/visitors and vendors.
