Position Summary:

This position acts as a coordinatorbetween internal and external referral sources, and acute inpatient rehaboperations. The Clinical Rehabilitation Specialist is responsible for establishingrelationships with case managers, physicians, payors, and other referralsources. The Clinical Rehabilitation Specialist will ensure appropriatepatient placement through clinical screenings and assessments in collaborationwith both hospital Case Management and the Rehab Medical Director.

The Clinical RehabilitationSpecialist works closely with hospital case management to ensure a responsiveand smooth flowing referral and admission process for the inpatient acute rehabprogram.

Position Requirements:
Minimum requirements:
  • Licensed Professional in a clinicalhealth care specialty (i.e. RN, Physical Therapist, Occupational Therapist, orSpeech Therapist).
  • Current state licensure in clinicalpractice area or eligible for state license transfer.
  • Graduate of an accredited school inclinical specialty.
  • Minimum 2 years recent clinical careexperience.
  • Professional appearance.
  • Effective verbal, written andpersuasive communication skills.
  • Computer and keyboarding skills (25wpm minimum).
  • Ability to work with others toensure quality patient care.
  • Organization and time managementskills.
  • Strong problem solving skills.
  • BCLS certification (AHA)
Preferred requirements:
  • Pass facility credentialing forFunctional Independence Measurement (FIM) within 90 days upon hire, thenmaintains current certification.
  • If RN, CRRN preferred and / orachieve within 2 years of accepting position.
  • Clinical experience in the care andmanagement of the Inpatient Rehabilitation patient.
  • Experience as a clinical liaisoncoordinating admissions for inpatient rehabilitation preferred.
  • Persistent and persuasive when facedwith challenging circumstances.
  • Ability to analyze data related toreferral opportunities.
  • Skilled strategy planning and goalsetting.
  • Ability to build strongrelationships with referral sources while being engaging, accountable andresponsive.
  • Self-directed with problem solvingcapability.
  • Ability to navigate a medical recordfor determination of medical necessity for an IRF admission.
  • Ability to analyze clinical datalooking at diagnosis and comorbidities that will demonstrate the patientscomplete medical picture.
  • Experienced in speaking in medicalterminology with physicians, case managers, therapists about disease states,comorbidities, and patient care.
