Hospice Education Coordinator

Position description
Collaborate with Organizational Learning Department, and Communications, other appropriate staff, and community members to develop and provide Hospice and bereavement educational programs/opportunities for the region based on identified needs. Assess educational needs of the community. Arrange workshops, seminars, educational programs for families and community caregivers based on educational needs. Catalogue and maintain current Hospice and grief literature for distribution to families and community members. Research current literature in Hospice and bereavement support to ensure current knowledge. Collect data to identify learning needs of community. Use specialized hospice education and experience to anticipate educational needs of community. Obtain input from community regarding educational needs related to Hospice and bereavement. Identify and prioritize actual learning needs of community based on a needs assessment. Assess audience characteristics related to readiness to learn, prior knowledge, maturation, and learning style. Plan and develop educational programs utilizing instructional and realistic behavioral objectives to meet the audiences learning needs. Produce and distribute educational events calendar, brochures/flyers, for intended audiences. Write objectives for educational programs that are in learner terms, identify the behavior desired and intended outcome. Utilize a variety of teaching methods and adapt methods to learner response. Provide adequate physical conditions (e.g. room arrangement, lighting, writing space, temperature, etc.) for optimal learning environment. Use audio-visual aids appropriately. Recommend to Director of Hospice budget needs based on education needs and projected goals and objectives. Implement educational programs according to established plan. Seek outside speakers or other knowledgeable sources as appropriate to meet the community needs. Ensure that information given to the community is current and accurate. Serve as liaison between Hospice, Community Health Services and other health care providers to provide optimal hospice care across the continuum. Assess and provide for educational needs to other health care providers. Communicate and coordinate processes to meet patient needs. Identify problems and facilitate solutions.
Experience: Requires a minimum of one to three years experience in this or related field. Must have or obtain a valid Minnesota drivers license. Experience with providing educational programs preferred.

Education: Baccalaureate Degree in Social Work, Psychology, Sociology, Education or related field required.

Must be able to work Independently
  • Requires travel within SWMN region
  • Knowledge of Hospice services and Medicare regulations preferred
  • Coordinates and tracks Hospice education to Long Term Care facilities and Assisted living as well as within our community
  • Serves as a liaison to our internal and external customers to improve communication and address questions/concerns with our Hospice services.
License or certification
Licensure & Certification: Drivers License required.

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $20.54 per hour.

Monday through Friday, day hours, with occasional evenings/weekends.

Weekend schedule
Occasional weekends.
