Duties

Primary Care Clinic LPN
This position is located at Mason City, Iowa

Using the Patient Aligned Care Team (PACT) Model, our clinic offers primary care to Veterans of all ages with a focus on putting the Veteran first as we demonstrate the ICARE (Integrity, Commitment, Advocacy, Respect and Excellence) values and continually work to improve clinic practices.
Alongside the Primary Care Provider, RN Care Manager, and Medical Support Staff; the LPN

  • Accurately completes a health promotion screening to include behavioral health and immunizations in preparation for the Primary Care Provider appointment;
  • Accurately documents and retrieves patient/clinical information to include vital signs and pain evaluation utilizing the electronic charting system;
  • Assists the PACT in screening and care of patients with acute illness;
  • Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes when appropriate;
  • Provides health education and coaching to the patient;
  • Assists in communicating follow up planning with patients;
  • Adheres to standards of practice and safe technique in preparation and administration of medicine;
  • Consistently practices ICARE values.
Work Schedule: Primarily Administrative hours Monday - Friday, but may include weekends

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Education: Must be a Graduate of a school of practical or vocational nursing approved by the appropriate State accrediting agency and/or the NLN at the time program was completed or completion of education or training in the military service which is accepted by the Licensing body in the jurisdiction in which the individual is licensed qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements
      • Licensure: Possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e. Puerto Rico) of the United States, or District of Columbia
      • English Language Proficiency: Must be proficient in spoken and written English (LVN/LPNs appointed to direct care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)

      Preferred Experience:
      • 1 year of primary care experience as a LPN.
      • Knowledge of PACT standards and concepts.
      • Demonstrated dependability for duty and flexibility in work assignment and hours in previous assignments.
      • BLS certification
      Grade Determinations:
      • GS-3 (Entry Level): No experience required beyond the basic requirements. LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.
      • GS-4 (Experience): Must have completed at least (6) six months of qualifying experience as an LPN or LVN.
      OR
      • GS-4 (Education): Must have graduated from an approved school and have (1) one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures.
      OR(Education Continued) Must have graduated from an approved school of at least 24 months duration.
      • GS-5:Must have completed at least (1) one year of qualifying experience at the GS-4 level.
      • GS-6: Must have completed at least (1) one year of additional qualifying experience at the GS-5 level or equivalent and fully meets all performance requirements for the GS-5 LPN.

      The full performance level of this vacancy is a GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of a GS-3 to a GS-6.

      Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, kneeling, pulling, and pushing. Transferring patients and objects may be required. The candidate may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The candidate may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The candidate will be working closely with others and may have protracted or irregular hours of work.
