Summary

The incumbent will serve as a Nurse Practitioner in Primary Care for the Patient Aligned Care Team (PACT).

  • Works with a multidisciplinary team including physicians, nursing, social work, dietician, clinical pharmacist, behavioral health and clerical support.
  • He/she will perform and record history and physical examinations and record data from clinic visit.
  • Evaluates, consults, develops diagnosis, and manages the primary care panel of patients assigned.
  • Enters progress notes in computerized medical record and completes workload documentation in a timely manner.
  • Provides medical care encompassing acute and chronic medical conditions and all appropriate preventive care for patients.
  • Order diagnostic tests, medications, laboratory tests as medically relevant for each patient.
  • Provides timely follow-up on results, including completion of view alerts.
  • Determines medical assessment and develop treatment plans according to accepted protocols.
  • Prescribes medication according to approved formulary, and in accordance with Federal and State Law.
  • May collaborate with an assigned mid-level provider and complete collaborative case reviews as assigned.

Work Schedule: Monday - Friday, 8:00 AM - 4:45 PM

      The full performance level of this vacancy is Nurse III. The actual grade at which an applicant may be selected for this vacancy is in the range of Nurse I to Nurse III.

      Physical Requirements: Heavy lifting, 45 pounds and over; Heavy carrying, 45 pounds and over; Use of fingers; Both hands required; Both legs required; Walking (2 hours); Standing (2 hours); Kneeling (2 hours); Repeated bending (2 hours); Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16" to Jaegar 1 to 4; Far vision correctable in one eye to 20/20 and 20/40 in the other; Ability to distinguish basic colors; Hearing (aid permitted).
