Practical Nurse, Woman's Health Clinic

Summary
Duties

The LPN/LVN provides basic and skilled practical nursing care. The nurse is responsible for observing, recording and reporting changes in the patient's condition. The nurse requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The nurse demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support, Medical Support Assistant (MSA) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patients. The nurse is responsible for, but not limited to, performing the following assigned functions:

  1. Provides basic and skilled nursing care and assistance to patients in the outpatient clinics, considering age, ethnic origin and special needs of the patient. May involve assistance with personal hygiene, lifting and positioning.
  2. Measures, reports, and documents vitals signs (i.e. body temperature, pulse, respiration, blood pressure, pulse oximeter), weight, height, blood glucose, and/or intake/output. Observes, reports and documents significant physical conditions, emotional behaviors and social needs as appropriate.
  3. Participates as part of the multi-disciplinary teams during care and treatment planning.
  4. Actively makes suggestions for change.
  5. Assists nursing staff and providers with maintaining clinic flow, completing assessments, setting up for and participating in treatments/examinations that require special skill, etc. according to medical center policies and procedures.
  6. Demonstrates competency in independently performing specific technical skills within the outpatient setting (i.e. complex dressing changes, urinary catheter insertion, endoscopy procedures, IV insertion/venipuncture).
  7. Administers medications in accordance with additional training, established policies and procedures. Observes, reports and documents changes in patient condition and/or behavior which may be related to medication administration.
  8. Assesses patient conditions, recognizes deviations from normal and notifies appropriate staff when immediate attention is necessary.
  9. Assists the RN with patient admission, transfers and discharge processes as appropriate to the outpatient setting.
  10. Appropriately documents care given in CPRS.
  11. Assists in maintaining and cleaning supplies and equipment according to facility policies and procedures and manufacturer recommendations. Restocks clinic areas as appropriate.
  12. Provides a safe environment and supports patient privacy. Reports unsafe equipment and conditions and adheres to infection control principles, emergency, safety and fire regulations.
  13. Demonstrates flexibility and willingness to adapt to changing work situations. Assists with problem solving.
  14. Is accountable for maintaining therapeutic interpersonal relations with patients, families/significant others and co-workers.
  15. Reinforces and reiterates instructions previously presented by the treatment team that provide the patient/family with appropriate health care information. Encourages patient participation in the treatment team by fostering self-reliance and independence and by involving the patient in their own treatment plan.
  16. Respects the patient as a person, accepts the patient and their illness and keeps personal information confidential.
Work Schedule: Monday through Friday, 8:00a.m.-4:30p.m.

    • Basic Requirements:
      U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      Education. Graduate of a school of Practical or Vocational Nursing, approved by the appropriate State Accrediting Agency and/or {accredited by} the National League for Nursing at the time the program was completed by the applicant.
      Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
      English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English.

      Preferred Experience: A minimum of 2 years full time recent LPN experience in Med/Surg, Ambulatory Care or Women's Health setting; basic life support.

      Additional Experience: refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Basic Life Support.
