HCA Houston Healthcare Northwest is a 423-bed acute care hospital that has been serving the north Houston community for more than 40 years, providing a wide range of medical services including cardiology, maternity care, neonatal intensive care, pediatrics, bariatrics and neurology.

HCA Houston Healthcare Northwest is affiliated with HCA Houston Healthcare, part of the HCA Healthcare Gulf Coast Division. The division is a comprehensive network of hospitals, outpatient surgery centers, emergency centers and diagnostic imaging facilities in greater Houston, Corpus Christi and South Texas. Facilities include: 17 hospitals, nine ambulatory care centers, 13 off-campus emergency centers, and a regional transfer center.

What you would do in this role:

Renal Dialysis Nurse is responsible for planning and providing patient care for the renal dialysis adult patients in accordance with the physicians orders, hospital policies and procedures, PSHA guidelines, AMI guidelines, JCAHO standards, federal, state, and local regulations.


The Renal Dialysis Nurse is responsible for communicating with the primary nurse prior to, during, and after the dialysis treatment as necessary to assure continuity of patient care. The Renal Dialysis Nurse is also responsible for acute and chronic treatment modalities as ordered. The Renal Dialysis Nurse independently performs crucial tasks in assessing, initiating, monitoring, and terminating treatment modalities in the inpatient and outpatient setting.

Performs duties independently, in a professional manner, without immediate or continuous supervision.


  • Follow and implement the dialysis treatment orders as ordered by the nephrologist and observe the policies and procedures.
  • Perform patient assessment, obtain, monitor, and vital signs.
  • Evaluate data and intervene appropriately. Perform Renal Replacement Therapies as trained and assigned
  • Administer medication, intravenous solutions, and document in the patients medical chart appropriately as per policy and procedure.
  • Provide and document pertinent patient and family education according to ESRD guidelines.
  • Assist proactively with the inventory needs; monitor the water quality treatment system, culturing, and disinfection of equipment following all infection control policies.
  • May assist with the education of the Critical Care team with CRRT.
  • Precept new team members as needed and submit accurate time records per Kronos in a timely manner.
  • Applies the nursing process to a group of patients. Evaluates outcomes of patient care, consults with other disciplines as required and adjusts the plan of care as necessary to ensure optimal service.
    Effectively identifies predictable problems and plans assigned tasks as needed.
    Performs nursing assessments and initiates the plan of care.
    Recognizes changes in patient condition and initiates activities to facilitate appropriate responses.
    Communicates the patients condition and/or change to appropriate staff and/or physician, as needed.
  • Ensure appropriate delivery of patient care for a group of patients according to policy and procedure; assigns and/or delegates patient care assignments to licensed and non-licensed nursing staff, based on acuity, needs of the patient, staff availability and qualifications.
    Makes patient care and related work assignments to the appropriate department staff; provides direction, and reviews all work, as necessary.
    Responds promptly to changes in workload, adjusting assignments as necessary to achieve optimal productivity and efficiency.
    Prioritizes and plans all work steps to ensure success; considers time, supplies and support required from other individuals and departments in setting up work plan and completing duties.
    Provides pertinent shift report to other members of the team.
    Assures appropriate follow-up of care for patients assigned to LVNs and for tasks delegated to non-licensed care givers.
    Participates and facilitates in the case management process and cost containment for patients.
    Makes appropriate referrals upon admission and throughout hospitalization.
    Considers all available options and makes appropriate decisions.
    Assists in the containment of hospital costs through appropriate use of supplies, recovery of patient charges, and conservation of supplies.
    Assists in the evaluation of patients for appropriate level of care and communicates with others on health care team.
    Communicates pertinent information regarding patients progress and discharge planning needs to appropriate hospital and medical staff.
    Evaluates patient progress in light of the plan of care and coordinates care to achieve desired outcomes.
    Cooperates with other personnel to achieve department objectives and maintain good employee relations with collaborative integration of the health care team.
    Maintains harmonious relationship with staff members, providing support and assistance to ensure quality customer service.
    Demonstrates attitude of cooperation and professionalism.
    Demonstrates flexibility in working hours and days scheduled in order to meet needs of the department.
    Seeks and accepts input from Directors/ Supervisors regarding interpersonal relationships that affect the departments harmony and serviceability.
    Demonstrates accountability for direct patient care and evaluation of patient outcomes.
    Follows established procedures for all treatments, performance of procedures, and medication administration.
    Assures the patient treatment plan is followed.
    Implements prescribed treatments accurately and in timely manner.
    Evaluates patient response/outcomes to procedures performed, treatments, medication administration and other aspects of the treatment plan.
    Recognizes changes in patient condition and initiates activities to facilitate appropriate responses.
    Documents all aspects of patient care efficiently and effectively.
    Ensures documentation and charging of resources/supplies.
    Documents complete patient assessment upon admission within time frame as per policy.
    Documents all medications, treatments, and evaluation of such.
    Determines and documents patient problems list, interventions and evaluation (PIE charting).
    Demonstrates professional practice and conduct at all times.
    Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
    Takes responsibility to regularly update self on knowledge of department and hospital communication and changes.
    Reports to work on time and absences are not to exceed hospital standards.
    Conducts job duties in accordance with established hospital, department, unit and Tenet policies and procedures and standards.
    Demonstrates leadership as team leader.
    Serves as a resource to co-workers.
    Participates in unit development and problem solving.
    Communicates effectively with members of health care team.
    Complies with legal and professional regulations.

    Demonstrates age specific competency for the geriatric
    Note sensory motor function changes and facilitate care according
    Assures dentures are in during use of ambu bag.
    Utilizes fall precautions
    Monitor for skin breakdown and provide care as needed
    Assess for an adequate swallow
    Assess physical and psychosocial changes associated with aging and make recommendations to other interdisciplinary care givers.
    Involve the patient in planning and providing self-care when possible and include primary at-home caregiver
    Demonstrates critical care skills for managing acutely ill patients.
    Demonstrates proficiency in identification and management of patients with dysrhythmias.
    Safely manages patients with mechanical ventilation.
    Demonstrates working knowledge of vasoactive and antiarrhythmic medications.
    Proficient in performing emergency protocols for RNs (ACLS)
    Demonstrates knowledge of specialized procedures performed in critical care area.
    Demonstrates Age Specific Competency for geriatrics:
    Adjust zone-aire bed for skin protection
    Assist with all ambulation due to physical limitations
    ECG leads applied to appropriate areas and changed daily to assess skin
    Involve family/guardian in patient education.
    Knowledge of age specific data
  • Participates in Department On Call rotation: Inpatient Registered Nurses demonstrates the ability to rotate through scheduled on call according to Department Guidelines. The On Call Dialysis Nurse will provide urgent inpatient renal therapies to the critical ill inpatient as scheduled by the Director.



Qualifications

Position Qualifications:


Minimum Education: See licensure requirement


Minimum Experience: 6 months to one year prior dialysis experience required


Required Licensure: RN Licensure in state of Texas


Required Certification: BLS


Skills: Ability to communicate with all customers both orally and written in a clear, professional and effective manner.
